NOTICE FOR PATIENTS 

From June 2018 there will be some changes at the practice:-

  • We are combining with Ladybarn Group Practice to provide services to our patients
  • Dr Chadwick is reducing his commitment and will do morning surgeries only
  • Other permanent doctors will provide afternoon surgeries and lady doctors will be available.
  • Management of the practice will succeed to Ladybarn Practice
  • We will have a new practice manager
  • Practice Nurse surgeries will not change
  • Reception staff will not change
  • There will be no changes to how you can :-
  • make appointments,
  • get your repeat prescriptions,
  • contact the surgery for any reason,
  • get out of hours medical attention 
  • get hospital and community medical services.

 OUR CQC INSPECTION  Thursday 30th July 2015

Many thanks to all our patients who gave up some of their time to take part in our Care Quality Commission inspection today.

All went well and we are very pleased to announce we received a good report.

News:

From Thursday 23rd July we will offer the Electronic Prescription Service

The Electronic Prescription Service allows your prescription to be sen electronically to a pharmacy of your choice. 

To sign up, you need to speak to your preferred pharmacy and complete a form.  Once signed up, your chosen pharmacy will receive your prescription electronically and you will not have to pick up your paper prescription from us.  If you have already nominated a pharmacy from Thursday 23rd July your prescription will be sent to them electronically unless you tell us otherwise. If you need to obtain a paper prescription for any reason, just let us know at the time of ordering.

This service is optional and you can continue to collect paper prescriptions as you do now if you prefer.  For more information please speak to your pharmacist.


 Patient On-Line-Access

The Patient Access service which is now available.

This will let you book and cancel appointments, request repeat medication and view your medical summary and immunisations record using the internet. This works alongside the current process for prescription requests.

All you need is access to the internet and an email account.

You will need to register with the service and create a user name and password for acces. Please  complete the application form attached (also available from reception) and return it to us with proof of ID & address.

Access

As this practice is situated in Burnage Community Healthcare Centre we have disabled access for vehicles within the car park and disabled access throughout the centre and GP surgery.
